- Delatestryl
- Depo Testosterone
- Fortesta
- Striant
- Testim
- Testopel
- Used the medication between the ages of 18 and 60
- Was hospitalized for one of the following while taking the medication
- Heart Attack
- Stroke
- Death
- Pulmonary Embolism
- Deep Vein Thrombosis
- Blood Clots
- All states except MI
- Has not retained legal counsel
