- Lineage : 2004 to 2012
- Dynasty : 2008 to 2012
- Profemur : 2003 to 2012
- Zimmer Biomet
- Durom Cup: 2006 to 2009
- MMC Cup: 2010 to 2011
- M/L taper Kinectiv: 2007 to 2015
- M2a 38: 2003 to 2012

- If unrevised, initial implant date from 2008 to Present
- If revised, initial implant date from 2001 to Present and revision within 10yrs of implant date

- Symptoms of injuries include :
- Catastrophic hip implant failure
- Dislocation
- Pseudotumors
- Tissue damage
- Osteolysis (bone damage)
- Reduced range of motion
- Loss of mobility
- Inflammation
- Infection
- Loosening or instability
- Noise from the hip joint
- Pain at the location of the implant
- Revision surgery
- Metal poisoning and/or symptoms, including :
- General hypersensitivity reaction (skin rash)
- Cardiomyopathy
- Neurological changes including sensory changes (auditory, or visual impairments)
- Psychological status change (including depression)
- Renal or Thyroid dysfunction
- No legal representation
